I have confirmation from Rolex Geneva that CPO warranty starts the day you buy the watch. So if you OQ stays 1 year in a drawer, and you buy it, it won't have lost 1 year of warranty. Don't worry.
Now, regarding OQ. I have one and had to pay a hefty price for a full service and motor.
Know two things: Battery change is free, for life (at least it is for me). I need to do this every 2-3 years.
Parts, there are few of them, and yes some of them are esoteric. I was told not to worry, unlike with AP, which has no possibility to repair old quartz ROs.
