Doesn't look right to me. Both the box and the watch itself.

I haven't found anything just like it yet via image search. It's part of an estate sale here in town. Huge house, very nice items for sale including a Mercedes SUV. I wondered if it could be a vintage watch in a new box? But again, I am completely ignorant about PP.

As far as I'm concerned Patek has never made a watch with only day, date and moonphase indicators, their watches would either have only the date, date and moonphase (in the case of the 5712, for instance), a month indicator for an AC and a leap year indicator when it came to PC (sometimes PC's won't have that indicator but they would still show the month). I'm afraid it doesn't quite look right for me either Lisa.
For what it's worth, Audemars Piguet has made a few watches with such a display of complications but I don't recall PP releasing such a reference.

Confirmed fake. Many thanks for helping a respectable estate sale proprietor save face.

We're here for that, I hope whoever bought this watch didn't pay too much for it...
